精準肝切除在肝膽管結石病的臨床應用_《中國普外基礎與臨床雜志》

目的評價精準肝切除術在治療肝膽管結石病中的作用,為肝切除治療肝膽管結石病提供新的理念。方法回顧性比較分析132例精準肝切除與52例非規則性肝切除治療肝膽管結石病在手術時間、術中出血量、術后并發癥、住院時間、結石清除率和住院費用方面的差異。結果盡管精準肝切除組患者手術時間〔(364.6±57.8) min〕長于非規則性肝切除組〔(292.9±44.7) min〕,P lt;0.001,但其術中出血量〔(558.3±90.6) ml〕明顯少于非規則性肝切除組〔(726.7±88.7) ml〕,P lt;0.001，術后并發癥明顯更少（11.4%比23.1%，P=0.004 3），結石清除率更高(89.4%比73.1%,P=0.005 5)。同樣，精準肝切除組患者住院時間〔(22.9±4.4) d〕短于非規則性肝切除組〔(28.8±3.5) d〕,P lt;0.001，住院費用〔(1.8±0.7)×104 元〕也少于非規則性肝切除組〔(2.1±0.9)×104 元〕,P=0.016 5。結論精準肝切除治療肝膽管結石病優于非規則性肝切除術。
