胸鎖乳突肌瓣預防腮腺切除術后味覺出汗綜合征效果的Meta分析_《中國循證醫學雜志》

目的　采用Meta分析方法評估胸鎖乳突肌瓣預防腮腺切除術后味覺出汗綜合征的效果。
方法　計算機檢索Cochrane圖書館、MEDLINE、EMbase、CBM、VIP、CNKI和WanFang Data中從建庫至2012年3月收錄的關于采用胸鎖乳突肌瓣預防腮腺切除術后味覺出汗綜合征的隨機對照試驗。由2位評價者獨立對符合納入標準的研究進行資料提取、質量評價并交叉核對后，采用RevMan 5.1軟件進行Meta分析。
結果　最終納入10個研究，共825例腮腺切除術患者。Meta分析結果表明：與空白對照組相比，采用胸鎖乳突肌瓣可使腮腺切除術患者術后味覺出汗綜合征主觀發生風險顯著降低78%［OR=0.22，95%CI（0.08，0.59），P=0.003］，客觀發生風險顯著降低83%［OR=0.17，95%CI（0.05，0.60），P=0.006］；敏感性分析顯示上述結果穩定性好。基于GRADE系統的證據等級均為“低級”。漏斗圖結果顯示無明顯發表偏倚。
結論　當前證據表明使用胸鎖乳突肌瓣能顯著降低腮腺切除術患者術后味覺出汗綜合征的主觀及客觀發生率。但鑒于納入研究的局限性，建議行更多大樣本、高質量的隨機對照試驗，并將是否影響胸鎖乳突肌功能作為結局指標之一做進一步論證。
