音樂療法在產程中應用效果的Meta分析_《中國循證醫學雜志》

目的　系統評價音樂療法在產程中的應用效果。
方法　計算機檢索Cochrane圖書館、PubMed、EMbase、EBSCO host、SpringerLINK Online Journals、CBM和萬方學術會議論文全文數據庫中關于音樂療法在產程中應用的隨機對照試驗，檢索時間為2000年1月至2010年12月。按照納入與排除標準選擇文獻、評價文獻質量和提取資料，而后采用RevMan 5.02軟件進行Meta分析。
結果　共納入9個RCT。其中有5個研究表明音樂療法可緩解產婦的疼痛感；有5個研究表明音樂療法可降低臨產后經陰道試產失敗轉為剖宮產的事件發生風險；有3個研究表明音樂療法可縮短第一產程時間；有2篇研究表明音樂療法能穩定宮口開全時的收縮壓和心率；還有3個研究表明音樂療法能緩解產婦的焦慮情緒。此外，音樂療法對新生兒Apgar評分無明顯影響；經敏感性分析，音樂療法對產后出血量影響的Meta分析結果不穩定。
結論　在產程中應用音樂療法能緩解產婦的疼痛感和焦慮，穩定宮口開全時的心率和收縮壓，并且降低臨產后經陰道試產失敗轉為剖宮產的事件發生風險，縮短第一產程的時間，對產婦的身心具有良好的效果。
