目的 描述并分析各國基本衛生服務內涵、服務包內容及遴選原則。
方法 由主題專家和檢索人員討論并試驗后確定檢索詞,共檢索22個電子數據庫、50個NGO及政府機構網站和搜索引擎Google,納入全世界所有描述及評價已經實施的基本衛生服務內涵、服務包和遴選原則、實施背景、策略及效果的英文文獻。用自行設計的數據提取表采集納入文獻的相關信息,而后進行描述和分析。
結果 共納入166篇文獻,其中104篇來自于中低收入國家的研究。其內容多是關于各國基本衛生服務包內容、遴選原則、實施背景與策略的描述(160篇),而涉及效果評價方面的文獻極少(6篇)。評價策略的研究大部分為橫斷面研究(5篇),只有1篇是有干預的時間序列研究。
結論 由于各國政治、歷史文化、經濟背景以及政策目標的差異,基本衛生服務政策的內涵、內容和遴選原則也各不相同。基本衛生服務是各國政府根據其政治、經濟和文化背景所采取的符合本國國情的公共籌資方式和實施策略,以達到人人可以公平享有的一攬子衛生服務項目。其服務包又可分為公共衛生服務和基本醫療服務兩部分,其中針對常見病和多發病的基本醫療服務包隨著各國社會經濟發展水平的提高在不斷擴大。基本衛生服務的內容需結合各國衛生體系和市場結構,以及供需雙方的實際情況;同時結合復雜的財政分配體系,組織和機構特點,以及國家調控結構和能力,綜合技術和社會價值兩方面的考慮,來確定優先服務領域。轉型國家的經驗尤其值得借鑒。
