二尖瓣對合面積和對合指數的實驗研究_《中國胸心血管外科臨床雜志》

目的　通過犬的動物實驗來研究二尖瓣出現中度反流的情況下二尖瓣對合面積及對合指數的變化趨勢，為預測遠期手術效果提供幫助。方法　應用飛利浦IE33超聲心動圖成像儀，對15只犬行經食管實時三維超聲心動圖（RT-3D-TEE）檢查，并建立動物實驗模型，在逐漸縮窄升主動脈造成左心室壓力升高的情況下獲取RT-3D-TEE圖像，直至二尖瓣出現中度反流為止。應用飛利浦三維定量Qlab7.0軟件的方法對原始數據進行后處理，計算對合面積和對合指數。動物實驗后計算心臟標本二尖瓣瓣葉的離體對合指數。通過受試者工作特征曲線（ROC）計算對合指數和左心室壓力的截斷值。結果　15只犬在無反流和有反流時對合面積［（198±50） mm2 vs. （123±36） mm2，P＜0.05］和對合指數（0.25±0.06 vs. 0.13±0.03，P＜0.05）差異均有統計學意義，對合指數和二尖瓣中度以上反流的ROC下面積為0.879±0.019， 95％ CI （0.843，0.916），截斷值為0.213 （P＜0.05）。左心室壓力和二尖瓣中度以上反流的ROC曲線下面積為0.882±0.021，95％ CI （0.840，0.923），截斷值為225 （P＜0.05）。離體瓣膜面積與舒張期總面積、離體對合面積與對合面積、離體對合指數與對合指數的差異均無統計學意義（P＞0.05）。舒張期總面積和離體瓣膜面積具有良好的相關性（r=0.937，P＜0.05），對合面積和離體對合面積具有良好的相關性（r=0.917，P＜0.05），對合指數和離體對合指數有良好的相關性（r=0.946，P＜0.05）。對合指數和離體對合指數的相關性高于對合面積與離體對合面積、舒張期總面積與離體瓣膜面積的相關性。結論　二尖瓣出現反流的情況下對合面積和對合指數有明顯下降，對合指數可更精確地反映二尖瓣反流的程度，為二尖瓣成形手術預后提供參考。RT-3D-TEE可以較為準確地測量二尖瓣對合面積和對合指數。
