目的　探討一個半心室修補術在雙心室修補術后急性右心功能不全治療中的應用。方法　回顧性分析2007年2月至2012年6月青島市婦女兒童醫院5例先天性心臟病行雙心室修補術后出現急性右心功能不全，急診在非體外循環下加做雙向Glenn手術（即轉換手術方式為一個半心室修補術）患兒的臨床資料，其中男4例，女1例；年齡7～18個月；體重6～13 kg。室間隔完整的肺動脈閉鎖1例，室間隔缺損＋房間隔缺損合并三尖瓣狹窄2例，法洛四聯癥合并三尖瓣狹窄2例。術后加強心肺功能維護，重點控制肺動脈壓。結果　急診雙向Glenn手術后72 h平均肺動脈壓12～18 mm Hg。呼吸機輔助呼吸時間3～182 h，ICU滯留時間2～13 d。住院期間死于肺動脈高壓危象1例。4例患者出院時靜息不吸氧狀態下經皮血氧飽和度為 93%～99%，較術前有不同程度地提高。隨訪4例，隨訪時間6個月至4年，心功能（NYHA分級） Ⅰ級3例，Ⅱ級1例。心臟超聲心動圖提示：腔靜脈肺動脈吻合口通暢，無血栓形成。結論　一個半心室修補術可作為雙心室修補術后發生急性右心功能不全的補救手術。
