目的比較 AO“分水嶺”萬向鎖定加壓鋼板內固定與外支架結合克氏針外固定治療橈骨極遠端骨折的臨床效果。方法回顧分析 2016 年 1 月—11 月，采取 AO“分水嶺”萬向鎖定加壓鋼板內固定治療的 9 例橈骨極遠端骨折患者臨床資料（內固定組），并與采用外支架結合克氏針外固定治療的 20 例患者（外固定組）進行療效比較。兩組患者年齡、性別、致傷原因、骨折分型、受傷至手術時間等一般資料比較差異均無統計學意義（P>0.05），具有可比性。結果兩組患者均獲隨訪，隨訪時間 5～15 個月，平均 10.7 個月。所有患者術后切口均Ⅰ期愈合，無感染、血管神經損傷、內固定物（內固定組）移位失敗等并發癥發生。兩組骨折愈合時間比較差異無統計學意義（t=0.891，P=0.391）；但內固定組住院費用顯著低于外固定組，末次隨訪時腕關節 Mayo 評分及上肢功能調查量表（DASH）評分顯著優于外固定組，比較差異均有統計學意義（P<0.05）。結論與外支架結合克氏針外固定相比，AO“分水嶺”萬向鎖定加壓鋼板內固定治療橈骨極遠端骨折具有內固定牢靠、便于患者早期功能鍛煉、術后腕關節功能恢復良好等優點。
