空心拉力螺釘結合側方支持鋼板治療LetenneurⅠ型及Ⅲ型Hoffa骨折_《中國修復重建外科雜志》

目的探討采用空心拉力螺釘結合側方支持鋼板治療LetenneurⅠ、Ⅲ型Hoffa骨折的療效。方法2004年5月－2011年4月，收治11例閉合性LetenneurⅠ、Ⅲ型Hoffa骨折。其中男6例，女5例；年齡25～47歲，平均36歲。致傷原因：交通事故傷8例，跌落傷2例，其他傷1例。左側7例，右側4例。按Letenneur等的股骨髁冠狀面骨折分型：Ⅰ型7例，其中外髁骨折6例、內髁骨折1例；Ⅲ型4例，其中外髁骨折3例、內髁骨折1例。新鮮骨折9例，陳舊性骨折2例。采用膝關節前內側或前外側切口，前后方向植入2枚直徑6.5 mm空心拉力螺釘并結合側方支持鋼板固定。結果術后切口均Ⅰ期愈合，無早期并發癥發生。11例均獲隨訪，隨訪時間12～26個月，平均15個月。X線片示骨折均達骨性愈合，愈合時間10～18周，平均15周。術后無內固定物松動及斷裂；均于術后9～15個月，平均12個月取出內固定物。末次隨訪時按Letenneur等的Hoffa骨折術后功能恢復評估標準評定，獲優7例，良3例，差1例。結論空心拉力螺釘結合側方支持鋼板治療LetenneurⅠ、Ⅲ型Hoffa骨折內固定可靠，骨折愈合率高；前外側或前內側入路是首選入路，尤其適合于合并脛骨平臺或髕骨骨折的患者。
