應用SurgiCase軟件指導游離腓骨皮瓣修復下頜骨缺損的研究_《中國修復重建外科雜志》

目的探討SurgiCase軟件在游離腓骨皮瓣修復下頜骨缺損中的應用價值。方法2010年9月－2012年3月，對10例病變切除后遺留下頜骨缺損患者采用游離腓骨皮瓣修復。男7例，女3例；年齡19～43歲，平均27歲。病變范圍7 cm × 5 cm～16 cm × 8 cm。術前行上、下頜骨和腓骨三維螺旋CT掃描，將數據導入SurgiCase 軟件中，模擬修復下頜骨缺損所需腓骨長度、位置并制作下頜骨重建后的三維頭模。術中截骨后下頜骨缺損范圍6 cm × 3 cm～16 cm × 5 cm；游離腓骨皮瓣修復下頜骨缺損，其中腓骨長6～17 cm，皮瓣切取范圍6 cm × 5 cm～16 cm × 6 cm。結果1例下頜骨造釉細胞瘤患者術中見腫瘤超過術前設計范圍，術前設計數據無法應用，根據具體截骨位置進行腓骨成型和修復；余9例患者均按照術前設計順利完成手術。手術時間5～7 h，平均6 h。術后切口Ⅰ期愈合，無早期并發癥發生。患者術后均獲1年隨訪。末次隨訪時，患者滿意度調查顯示8例患者對面型滿意，2例自覺患側面型較寬。曲面斷層片顯示骨愈合良好，未見骨不連；開口度2.5～3.5 cm。結論在游離腓骨皮瓣修復下頜骨缺損中應用SurgiCase軟件進行術前設計，能為腓骨截取及下頜骨修復提供精確數據，提高了手術精確度。
