Objective To conduct a visual analysis of domestic and international research on elderly constipation nursing, explore the research trends, hot topics and developmental context in this field, and provide references for future nursing practice and research. Methods Research literature was retrieved from China National Knowledge Infrastructure and Web of Science Core Collection, with the retrieval period ranging from 2005 to 2025. Visual presentation and interpretation of hotspots and research frontiers of domestic and foreign studies on elderly constipation nursing were performed using CiteSpace software through keyword co-occurrence analysis, cluster analysis and burst detection analysis. Results A total of 1002 Chinese articles and 517 English articles were included. The analysis results indicated that domestic research hotspots focused on traditional Chinese medicine nursing and its external therapies, including acupoint application, auricular point pressing, acupoint massage and so on. In contrast, foreign research concentrated on disease management, prevalence, quality of life and medication-related issues. Keyword burst analysis revealed that the research frontier in China had further narrowed down to refined clinical applications of traditional Chinese medicine external therapies, while foreign studies had shifted toward clinical research areas such as diagnostic optimization, stratified management of elderly patients and nutritional support. Conclusions Domestic studies highlight the practical pathway of “symptom-intervention-outcome”, with a greater focus on nursing interventions and traditional Chinese nursing approaches. Foreign studies adopt an evidence-based model of “etiology-management-prognosis”, placing more emphasis on management strategies, risk factors, diagnostic optimization and improvement of quality of life. Future research on elderly constipation nursing should strengthen interdisciplinary collaboration, attach importance to precise assessment and formulation of individualized nursing protocols. Meanwhile, it is necessary to promote the development of integrated traditional Chinese and Western medicine nursing, especially the standardized application of external traditional Chinese medicine therapeutic techniques.
