Objective  To investigate the current status of adherence to the Chinese Expert Consensus on Perioperative Nursing for Transcatheter Aortic Valve Replacement (hereinafter referred to as the “Consensus”) in clinical practice and its influencing factors, so as to provide evidence for promoting its clinical application. Methods  From July to October 2025, a questionnaire survey was conducted among 255 nurses from hospitals at all levels performing transcatheter aortic valve replacement across 22 provinces in China. Adherence percentages to the Consensus were obtained for 19 recommendations, and logistic regression analyses were performed to identify the influencing factors. Results  Among the 19 recommendations in the Consensus, adherence was high for intraoperative defibrillator management (93.08%), team building (91.37%), anesthesia management (90.98%), coordination of and participation in intraoperative thoracotomy (84.62%), and dedicated intraoperative valve table management (83.85%); low for management of patients at high risk for postoperative stroke (35.38%), intraoperative temporary pacemaker management (27.56%), postoperative support for family members and patients (18.92%), intraoperative temperature management (8.46%), and nutritional assessment (0%); and moderate for other recommendations (50.00%-77.03%). Logistic regression analyses showed that, compared with nurses unfamiliar with the Consensus, those with high familiarity had significantly higher adherence to team building [odds ratio (OR)=37.417, 95% confidence interval (CI) (4.206, 332.834), P=0.001] and rehabilitation management [OR=14.120, 95%CI (1.632, 122.146), P=0.016]. Compared with nurses with low competence in implementing the Consensus, those with high competence exhibited significantly higher adherence to coordination of and participation in intraoperative thoracotomy [OR=19.662, 95%CI (1.731, 223.390), P=0.016] and postoperative deep vein thrombosis prophylaxis [OR=8.551, 95%CI (1.182, 61.865), P=0.034]. Nurses who perceived the application of the Consensus as highly meaningful showed significantly higher adherence to intraoperative anticoagulation management [OR=7.399, 95%CI (1.671, 32.752), P=0.008] and postoperative transfer to the unit and neurological monitoring [OR=11.152, 95%CI (1.373, 90.591), P=0.024] than those who considered it less meaningful. Additionally, nurses with longer work experience, senior professional titles, and female nurses demonstrated significantly higher adherence to rehabilitation management, dedicated intraoperative valve table management, and intraoperative restraint management, respectively. Conclusions  Adherence to the Consensus is uneven and significantly influenced by familiarity with the Consensus, competence in implementing the Consensus, perceived meaningfulness of its application, as well as years of work experience, professional title, and sex. In the future, targeted interventions can be developed based on these influencing factors to promote the clinical application of the Consensus.
