目的  探討圍手術期強化免疫營養素腸內營養支持對胃癌患者免疫功能和營養狀況的影響。 方法  將2007年3月-2012年12月間24 例胃癌術后患者隨機分為腸內免疫營養組(研究組)和常規腸內營養組(對照組),每組12例,進行等氮等熱量營養支持,術前5 d至術后7 d。測定營養支持前后的血清白蛋白、前白蛋白、轉鐵蛋白、T淋巴細胞亞群及血清免疫球蛋白等指標變化。臨床觀察胃腸功能恢復時間。 結果  術后兩組患者血清蛋白水平和免疫指標多有下降,但研究組術后8 d的血清前白蛋白水平和IgG、CD4/CD8比值較對照組明顯增加,研究組胃腸功能恢復時間早于對照組。 結論  圍手術期應用強化免疫腸內營養支持可改善胃癌患者術后營養和免疫狀況,促進腸道功能恢復。
